I have a guest who wanted to ‘add’ a day to their reservation. I sent a change request, but they have not responded yet. Does this ‘block’ that day until change is approved? I am concerned that the request change will not happen for a day or so, and my day will be blocked for potential other bookings.
When you send over the request I would have said to the guest please confirm within 4 hours if you want to go ahead with the amended booking or I will withdraw the offer.
I always tell a guest if they want to amend the booking they should do so to avoid being in this position. @Rolf

No, it doesn’t. It is open. You can verify this by looking at the listing’s page and clicking on that date.
Just remember that if they make the change request and you accept it the 48 hour/14 day clock starts again, and that is a well documented way for guests to cancel penalty free in some instances.
And that’s why I don’t accept alterations. To add a date I ask them to send a request to book and then I send them a special offer minus the cleaning fee.
That’s the way to do it.
